Doctor Saiki Tobio is a rich handsome playboy, and is much liked by his female patients and nurses. However, whenever he has a lewd thought, he transforms into a fat and ugly man. In his alter ego, Tobio goes by the name of Sasaki Kintarou. Takami Moeko is a nurse who had just transferred to Tokyo, from a small rural town. She works alongside Tobio at the hospital and is disgusted by his heartlessness. One day she gets to know Kintarou, and is very much taken with his sincerity, not knowing that he is actually the altered Tobio. Slowly but surely, they start to develop a relationship...
